Does My Child Qualify?

To be considered for admission, your child should fulfill the following requirements:


Patient Referral Form

  • Medical Complexity or Fragility, and/or Technological Dependence:

    Your child should demonstrate medical complexity, fragility, and/or technological dependence.

  • Doctor's Approval:

    Approval from your child's doctor is a prerequisite for admission.

  • Medical Stability:

    Your child must be medically stable, without an immediate need for hospital care.

  • Suitability for Outpatient Care:

    The condition of your child should be suitable for outpatient care.

  • Need for Skilled Nursing Care:

    Admission is reserved for children who require skilled nursing care.
